Cost: 18 and younger, we will bill insurance. For those without insurance, there will be no cost. Adults, we can bill most commercial insurance or it is $20.00.

If you would like to have your child receive the flu vaccine, a consent form will need to be filled out and returned to school by Thursday, September 28th.

Parents, if you wish to accompany your child, we ask that you get to the school at the start of the clinic and bring your child's consent form with you.

If over half of our Crockett students get their flu shot from the clinic this year, EVERYONE will get an extra recess!

Meet the Teacher is Thursday, August 24th from 5:30 - 7:00 p.m. Bring your students to meet their new teachers, drop off school supplies, and explore different stations around campus. If you are interested in serving on the Crockett Elementary PTA Board for this upcoming year please fill out the attached form and return to the office by Friday, May 12th. You may nominate yourself and/or others for a position. General Assembly Meeting to vote for the 2017-2018 Crockett PTA Board Members will be held Monday, May 22nd at 3:45 p.m. in the Cafeteria.
