Flu Shot Clinic

Date: Friday, September 28, 2018
Time: 7:15 a.m.
Cost: 18 and younger, we will bill insurance. For those without insurance, there will be no cost. Adults, we can bill most commercial insurance or it is $20.00.

If you would like to have your child receive the flu vaccine, a consent form will need to be filled out and returned to school by Thursday, September 27th.

Parents, if you wish to accompany your child, we ask that you get to the school at the start of the clinic and bring your child's consent form with you.
